Job Code (Position #): 1447 Current Pay Range: $36,976 - $40,405 Work Hours: M-F 8-5 # of Openings: 1 JOB PURPOSE: Develops the individual treatment plan to meet needs of adult mental health consumers as determined by the Adult Strengths and Needs Assessment (ANSA) and Child Assessment of Needs and Strength (CANS). Performs additional assessments as needed for completion of the ANSA and CANS. Provides case management and skills training services to consumers in accordance with the individual treatment plan. Uses information obtained through observation of consumer and provision of service to make changes to treatment plan for consumer. Completes all paperwork necessary to document services performed. EDUCATION AND EXPERIENCE REQUIRED: A bachelor’s degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse.
