We are seeking an experienced mid-level Provider or Physician to join our team! Provider is responsible for performing physical exams, ordering appropriate lab tests, and evaluating those tests, to ensure quality care of the patient. The Provider is also responsible for electronic medical record entries which reflect the patient's healthcare within the allotted time frame. This is an independent contracting position staffed during the week days. The ideal candidate will have 1 up to 3 days per week of availability and the ability to travel, as needed, throughout the state. The Nurse Practitioner/Physician Assistant must be well versed in their ability to perform ALL of the following exams: Range of motion (using a goniometer) Diabetes (monofilament) PAP Pelvic Rectal Smell/Taste Test Cognitive Screenings Musculoskeletal If you are made a conditional offer of employment, you will be required to undergo background check (including criminal record check) and drug screening....
