Join our dynamic team of case managers for our Nassau Case Management program. Case Managers be responsible for assessing the client and client's support systems, complete screenings for benefits/entitlement programs, determine cost shares, and develop a comprehensive case plan. They will authorize and arrange for services, refer and link clients to community resources, and monitor and follow up on services delivery and the client's status as required by state guidelines. Selfhelp complies with all Federal, State and Local laws in regards to wages. Knowledge, Skills And Requirements Bachelor's in Social Work or related field. Knowledgeable about entitlements and benefits. Must have car to travel.
